Where Are My Incisions Located When I Get BBL?

A Brazilian butt lift (BBL) combines the artistry of body contouring with precise surgical techniques to enhance your natural curves. While many patients focus on their expected results, understanding where your incisions will be placed can help you feel more confident about your procedure. At PURE Plastic Surgery, Dr. Alexander Earle and our surgical team strategically position tiny incisions to minimize visible scarring while achieving beautiful, balanced contours.

BBL Incision Sites

During a BBL, your surgeon harvests fat from one area of the body and injects it into your buttocks to increase its size and shape. Using advanced ultrasound guidance technology, the surgeons at PURE Plastic Surgery can see exactly where the fat is being placed, ensuring optimal results while maximizing your safety.

While this process does require incisions, they are strategically placed to reduce visible scarring, allowing you to confidently wear swimsuits and form-fitting clothing after you've healed. Below are the most common incision sites for injecting fat into the buttocks.

Near the Top of the Upper Buttocks

One common BBL incision site is near the top of your buttocks, where it can be easily hidden by clothing, undergarments and swimwear. This location allows your surgeon to precisely inject fat into the upper portion of your buttocks, creating the sought-after lift and projection that defines an attractive silhouette.

Beneath the Gluteal Crease

Another place where BBL incisions can be made is beneath the gluteal crease — the natural fold where your buttocks meet your thighs. This location serves two purposes: it provides access for optimal fat distribution in the lower portion of your buttocks while remaining hidden in a natural body fold, even when wearing revealing clothing.

Hidden Between Both Cheeks

Depending on your needs, your surgeon may decide to put your incisions between your buttock cheeks. This central placement enables your surgeon to achieve symmetrical and natural-looking results while keeping any potential scarring completely concealed.

Liposuction Incision Locations

Patients love BBLs because they transfer unwanted fat from one area of the body into another to sculpt a curvier, more defined figure. To achieve this, excess fat is removed using a small cannula inserted through tiny incisions. The number and placement of these incisions vary based on your unique anatomy and goals, but they're always positioned with future scarring in mind.

In the Abdomen

The abdomen is one of the most popular donor sites for a BBL, allowing patients to achieve a flatter tummy while enhancing their natural curves. Small incisions are typically made in the lower abdomen or belly button, ensuring they remain virtually invisible once they’ve healed.

In the Thighs

When collecting fat from the thighs, incisions are typically placed near the groin, in the natural crease where your leg meets your body. This careful placement allows your surgeon to access pockets of unwanted fat while ensuring any resulting marks remain well-concealed.

In the Hips

Some patients prefer to have excess fat removed from the hips to eliminate the appearance of “saddlebags,” while others opt to have fat injected into this area to fill in hip dips. Either option can help you achieve a lovely hourglass shape. Incision sites in this location are typically placed just above or on the sides of the hips, depending on your specific needs and goals.

Does a BBL Result in Visible Scarring?

When undergoing a BBL, some incisions are needed to harvest unwanted fat so it can be transferred into the buttocks. Generally, you can expect between two and five liposuction incisions, depending on how much fat is being removed (and from where). Additional incisions will also be made in the buttocks to administer the purified fat and sculpt a curvier figure.

Thanks to advanced surgical techniques and the expertise of Drs. Earle, Vidal and Wegerif, BBL scarring is minimal. The incisions used for liposuction are extremely small — typically just a few millimeters in length — and with proper aftercare and sun protection, they fade significantly over time. Similarly, the prepared fat is injected into the buttocks using a very small cannula, resulting in tiny, dot-like marks. Once they’ve healed, they’re barely noticeable, even in a bikini!

Introducing the South Beach Butt Lift (SoBeBL)

The SoBeBL is Dr. Earle's signature BBL technique, developed through years of experience. This innovative procedure uses fewer incisions than traditional BBLs, resulting in less visible scarring while still achieving stunning results. The SoBeBL emphasizes natural-looking enhancement that complements your existing body proportions, creating the smooth, elegant contours that have become known as the "Earle Curve."

Post-Op Care: Getting the Most Out of Your BBL

Proper incision care plays a vital role in your BBL recovery and results. While the strategic placement of incision sites helps minimize visible scarring, it’s important to follow all pre- and post-op instructions provided by your surgeon. For the first few weeks after surgery, your incisions require special attention to prevent infection and promote proper healing.

Keep Your Incisions Clean and Dry

Maintaining cleanliness around your incision sites helps prevent complications during recovery. Our team will provide detailed instructions on how to clean your incisions, including which products to use and how often. While showering is typically permitted 48 hours after surgery, you'll need to pat the areas dry gently rather than rubbing them.

Wear Your Compression Garments

Wearing your compression garment as directed does more than support your new contours – it also protects your incisions and promotes proper healing. This specialized garment helps reduce swelling and ensures your skin adheres smoothly to your new shape.

Protect Your Incisions from Sun Exposure

Sun exposure can cause incision sites to darken, making them more noticeable. Once your incisions have healed, apply broad-spectrum sunscreen to any exposed areas. This simple step helps prevent hyperpigmentation and ensures your scars fade as much as possible.

Reducing Scarring With Nonsurgical Treatments

If you’ve taken every precaution but still have some visible scarring after your procedure, nonsurgical treatments like Morpheus8 and IPL can help. Morpheus8 combines radiofrequency energy (RF) with microneedling to improve skin texture, reduce discoloration and create a more uniform appearance, making it ideal for surgical scars and stretch marks. IPL is a light-based treatment that can significantly reduce the redness and visibility of incision sites and scars.
